CARE MANAGEMENT

Care Management activities for consumers include a comprehensive needs assessment, a written care plan, arrangement for the follow-up of service delivery and reassessment, when coordination of services is needed.

HOME DELIVERED MEALS *

Home-Delivered Meals are provided to eligible home-bound seniors.  The program targets nutritionally at-risk individuals who are unable to prepare nor secure meals.  Meals are delivered through subcontracted agencies.

PERSONAL EMERGENCY RESPONSE SYSTEM (PERS)*

The PERS is a pendant that can be worn on the wrist or around the neck that allows a consumer to activate a button to call for help if needed.  The PERS is available under an approved plan of care for consumers who are assessed to be at high risk and are able to cognitively and functionally be able to use the PERS.

PERSONAL CARE & RESPITE*

Personal Care Aides assist with bathing, dressing and grooming, and also provide caregiver relief.  In all situations, in-home services are preceded by an extensive Needs Assessment of the individual by a certified care manager.

CONSUMER REIMBURSEMENT *

Provides reimbursement to consumers who wish to purchase their own in-home services as part of an approved plan of care. 

*Provision of services is dependent on the availability of funds
The senior may be required to pay a portion of the cost of the in-home services. Costs are determined using a sliding scale. Low-income seniors are not charged.
